Population Health Management (PHM) is an innovative model of care delivery that aims to improve the health of patients while lowering the overall costs of care. The Population Health Analyst provides advanced technical and analytical support for Signature Medical Group providers. The work assignments and responsibility require in-depth knowledge of financial and clinical analyses and cost and utilization trends related to payors, employers and health plan data. Responsibilities include, but are not limited to, utilizing databases and reporting tools to extract data, generate reports, perform complex data analysis and draft system/process improvement recommendations. Collaborate with key stakeholders to gather and analyze needs and requirements: design and implement robust analytic and reporting solutions to support identified needs. Organize, assemble and analyze data and key performance indicators to better understand the performance of departments, clients and organization initiatives. Conduct trend and comparative analysis of key data. Prepare monthly reports of indicators across all departments. Participate in the review and recommend changes for improvement. Construct and deliver written reports of the analytic findings in a variety of formats including visualization of data, The Analyst focuses on assessing the implementation and effectiveness of strategies meant to efficiently improve the health of the diverse patient population aligned to Signature Healthcare's primary care physicians.
