Leveraging the wide array of CVS Health assets, we will disrupt this stagnant industry and need a new Manager Care Management Portfolio & Scaling Operations to help us reach this goal.The Manager of Care Management Portfolio & Scaling Operations must approach problem solving with a deep level of curiosity and the ability to think creatively to solve operational challenges, while executing in a fast-growing environment. The Manager of Care Management Portfolio & Scaling Operations will be accountable for scaling capabilities and integration with our enterprise assets after being successfully piloted. The Manager of Care Management Portfolio & Scaling Operations will also be accountable to drive key operational projects that will improve clinical outcomes, increase access to home dialysis, and drive financial performance in our care management and risk products.

Required Qualifications

2+ years of prior work experience managing operational KPIs in a healthcare setting.5+ years of Project Management experience.

As a part of this process, you will be required to provide information or documentation about the reason you cannot be vaccinated. If your request for an accommodation is not approved, then your employment may be terminated.

Preferred Qualifications

1+ years of experience in population health operations in an at-risk environment.1+ years of experience managing a population with Chronic Kidney Disease.
